Maintenance process: from work request to root cause failure analysis

Maintenance is the cycle that keeps plant running: a request or a due date becomes a work order, the work is planned against parts and a downtime window, done as preventive or corrective work, tested back into service and closed with a costed record. The templates below follow that cycle in order, with the unplanned and the specialized cases alongside.
Every job starts as a request in the work order process: screened as valid, typed and prioritized, then held until parts are committed and the weekly schedule has a slot. Scheduled work follows preventive maintenance, where an interval or meter trigger becomes a planned work order, a downtime slot and a tested handback. A defect on a still-running asset follows corrective maintenance instead: triage, a safe-to-run decision, job planning, a booked window, a tested repair and a work order closed with a failure code. The equipment maintenance chart shows how those routes, and a condition-monitoring alert, converge on the same CMMS record and the same acceptance back into service.
The plan fails at its edges. An unplanned stop bypasses scheduling: the breakdown and troubleshooting charts make the machine safe, start the downtime clock, try a first-line fix within operator scope, then repair or escalate. A failure that meets the investigation criteria should not close with its work order; the failure investigation preserves the failed parts, confirms the root cause against the evidence, gets the corrective actions funded and monitors a verification period that proves they worked. Predictive maintenance turns a reading over the alert limit, confirmed by a second technique, into a repair planned before the failure.
Calibration, the planned shutdown and the food line's hygiene release are the specialized cases, with two SOPs beside them: preventive maintenance and calibration planning for instruments under a service agreement, adjusting intervals from drift trends, and generator and electric motor maintenance for rotating machinery, with its vibration-analysis call and test-run gate. Maintenance sits between two other families. Commissioning and decommissioning are in the asset lifecycle family; the isolations, permits and gas tests that make intrusive work safe are in permit to work. Both share the maintenance lane, so a work order here and a permit there describe the same job from two sides.
