Safety incident process: near miss, incident reporting and investigation
Three templates for the health and safety reporting loop: near miss reporting, workplace incident reporting with statutory notification, and investigation to verified controls, plus the emergency decision tree and two inspection rounds.
Safety incidents is the reporting-and-learning loop of a health and safety system: a close call or an injury is made safe and reported, classified and notified where the law requires it, investigated past the person to the conditions, and closed with corrective actions verified in the workplace. Three templates form the loop; the emergency decision tree and the two inspection rounds feed it from either side.
The loop starts before anyone is hurt. Near miss reporting takes a close call from the person who saw it: made safe, reported by name or anonymously, rated for the harm it could have caused, not the harm it did, then answered back to the reporter with what changed, which is what keeps reports coming. When someone is injured, workplace incident reporting adds first aid, the statutory report for a serious injury or dangerous occurrence, and the handoff into investigation. The investigation then preserves the scene, secures evidence and witnesses before they decay, traces the causes past the person to organizational factors, and verifies the controls where the work is done.
The usual failures are silence and blame: near misses that go unreported because nothing came of the last one, investigations that stop at operator error, and corrective actions assigned but never checked. Each chart has a step against one of them: the answer to the reporter, the cause traced to organizational factors, and verification in the workplace rather than on the form. The first minutes of an emergency are a different problem. The emergency response decision tree asks who is at risk, whether the area is safe to approach, whether to evacuate or shelter, and who gives the all-clear, each in the lane of whoever answers it; the incident report begins once the emergency is over.
Inspection feeds the loop from the other side: the safety inspection and the joint workplace inspection rounds find hazards before they become incidents, stop a task on imminent danger, and route each finding to a named owner with a due date and a fix verified on site. The controls that should have prevented the incident are the permit to work family; where a corrective action is systemic rather than local, it joins the CAPA loop.
