"Issue maintenance report with test and alignment data" is only useful as a baseline for the next visit if it actually carries the before/after insulation resistance readings, vibration spectra and alignment figures. List what's mandatory in the row's comment field so the report isn't a pass/fail line with the numbers left out.

Does every generator or motor visit get a vibration analysis?
No. "Vibration analysis required?" is a real decision, not a step run on every visit: it's warranted on larger or critical machinery, a unit with a history of vibration complaints, or when the electrical and mechanical tests didn't fully explain a reported symptom. A small motor that tests clean on a routine visit can skip straight to "Diagnose fault and confirm repair or overhaul scope".
What happens if the repaired unit fails its test run?
The "Test run within acceptable limits?" decision routes a fail straight back to "Repair or replace windings, bearings and electrical components" rather than logging it as a noted exception. A generator or motor that runs hot, rough or noisy on the bench does the same thing back in service, only with the customer depending on it by then, so the chart treats a failed test run as unfinished repair work, not a caveat on the report.
