Sample transport process flowchart

An editable sample transport workflow for defining transfer conditions and documents, preparing a package and handoff record, assessing arrival condition, escalating exceptions and closing the transfer record.

What the sample transport process is

This workflow maps a sample transfer from an initial request through packaging, handoff, receipt and a recorded outcome. It is designed to be adapted for the routes, packaging instructions, records and roles that your organization uses.

The chart keeps two exceptions explicit: an unclear transport plan pauses the request for clarification, and an arrival condition that needs assessment is documented and escalated rather than silently accepted.

This template does not establish compliance with any standard or regulation. Confirm the packaging, safety, security, transport conditions, custody records and authorization roles that apply at your site before making it effective.

What this flowchart covers

In this template

  • Four stages from transport planning through a completed transfer record.
  • A labelled decision for defining transport conditions and documentation before preparation.
  • A handoff-and-arrival assessment that separates acceptance from exception handling.
  • A route for holding an unclear request or escalating an arrival finding.
  • Editable sender, courier and reception swimlanes.

When to use this template

  • You transfer samples between collection sites, laboratories, storage locations or testing areas.
  • You need to make packaging, transfer records and receiving assessment visible in one process map.
  • You are defining what a receiver does when the sample or transfer record arrives with an exception.

How it works

  1. Define transport plans

Add the routes, handoff points, packaging instructions and records your laboratory uses. Keep service levels and local conditions in the approved procedure rather than treating them as universal.

  2. Set sender and receiver checks

Specify what the sender confirms before release and what the receiver checks at arrival. Connect each check to the record or system that captures it.

  3. Map the exception assessment

Name who owns an arrival concern, what is held while it is assessed and how the decision reaches the sender or request owner.

  4. Test a live handoff

Walk a normal transfer and an exception through the chart with the teams at both ends. Add any real courier, storage or record handoff the draft misses.

Frequently asked questions

What does a sample transport process cover?

It covers planning a transfer, preparing the package and record, handing the sample over, assessing its arrival and either accepting it or escalating an exception before the transfer record is closed.

What should happen when transport conditions are not defined?

Pause preparation and clarify the transfer plan with the responsible owner. A request without a clear route, record or handling instruction should not be converted into an improvised handoff.

What should a receiving team do when an arrival looks unusual?

Document the observation, keep the sample in the controlled status your procedure requires and route the assessment to the appropriate owner. The receiving team records facts rather than making an unsupported disposition.
