    Add links or notes for the performance records, change records, issue logs and method versions that reviewers consult. This makes the chart a map of evidence rather than a generic meeting sequence.

  3. Define lifecycle actions locally

    Specify the possible actions and decision owners used at your site, such as further assessment, method update, restricted use or retirement. Do not treat the examples as mandatory outcomes.
